Have a friend shoot it with an iPhone and it won’t cost anything. You’ll need to plan the sequence of shots carefully — a shooting script — and rehearse how it will be shot several times. Stabilization for the iPhone will be a must. Be sure to light the working space evenly and check out how you look on camera.
It you want to have it professionally lighted and shot I would expect to pay at least $300 to $500 for a simple shoot, more if you require closeups and cutaways, plus the cost of editing, adding titles, etc. $500 to $700 total would be a reasonable expectation.
