My best guess is most of it is real camera work. The surface looks like glass, frosted on one side and most of the sparkle just looks like well positioned lights. There are a couple when the gem is in front that look like trapcode starglow in AE or something of the like to enhance things and make it fuller. Half the fun of doing these things is figuring them out. Let us know how it goes.
