DVD Slim Case Template

   Does anyone have a template for Adobe photoshop for A4 DVD slim case inserts. I have one for the regular size cases of course but not for the slim ones. I tried google and could not find one just to download. Thanks.

   I make all of my own templates but I haven't made one for slimline cases.
